To get started on your new Web site we need a little information from you.
- Please open the Design .PDF (Click the blue button to the right)
- Complete the .PDF and save it to your desktop with your LAST NAME INCLUDED in the file name for example (Dempsey-design-order-form)
- Complete the questionnaire below
- Upload the Design questionnaire and any corresponding logo files or images you would like used on your site. Please make sure your LAST NAME is included in all the image filenames. For example (Dempsey-logo.jpg)
- All websites require a $500 deposit. If you can download the Credit Card Authorization Form here or pay by check.
Please Complete the form below. This Questionnaire will take 15 to 20 minutes to complete, but is necessary for us to develop your Strategic Site Report creating the road map to make your site successful.
